JOB SUMMARY
The Social Worker MSW 1 PRN provides patients and family members with the education and advice needed to cope with issues which arise from various emotional or physical problems as outlined by the Texas State Board of Social Worker Examiners scope of practice. Coordinates a variety of services including crisis intervention, bereavement, client advocacy, referrals, prevention, education, discharge planning and implementation, and psychosocial assessments. Collaborates in the development and implementation of performance improvement initiatives.
ESSENTIAL FUNCTIONS OF THE ROLE
Conducts and documents detailed psycho-social assessments to interpret social, environmental, and medical needs of the patient.
Employs advanced practice methods in the development, implementation and evaluation of treatment plans. Provides patient and family education and serves as a professional resource.
